Office Policies:
We have instituted necessary policies in our practice that enable us to provide the best care possible.
Adhering to these policies has proven to be beneficial to every patient and has allowed
us to maintain timeliness and efficiency in our practice.
 
*Co-pays are to be made at time of service.  Any appointment is subject to cancellation/reschedule if copayment is not made (exceptions possible)
 
*We appreciate cancellation notification within 24 hours of the appointment if possible.  Any patient that misses 3 appointments without calling ahead to cancel may be subject to dismissal from practice.  Re-entrance into the practice is possible.  Email office manager at akraft@bucyrusinternalmed.com with any questions regarding this policy.  "No show" appointments will be subject to a $20.00 fee that is not subject to insurance submission.
 
* If you are 20 minutes late for an appointment without calling ahead, appointment may be subject to cancellation and may be rescheduled at a later date (extenuating circumstances considered).
 
* Prescriptions re-fills will be completed by 5pm on the next business day.  Please inform your pharmacy of the re-fill being requested so that they can contact our office with the correct medication information and instructions.
 
* Any form (ie: insurance or disability, FMLA) needing to be filled out will be completed within 5-7 business days.  If a form is presented without an appointment, there will be a $20 charge for completion.
 
* Answering machine messages are checked daily.  If in the event of an emergency, you may reach Dr. Harris after hours - leaving an urgent message by following the instructions on the message ("press # for further option, mark message as urgent").  This feature is for emergencies only.  As always, if you have an emergency, please go to your local emergency room or urgent care facility.
 
* Our office typically does not handle work-injuries and/or claims through the Bureau of Workers' Compensation.
